Independent Living Skills Assessment/Goal Setting
Person to Person’s nationally certified and state licensed Occupational Therapist (OTR/L) will administer two independent living skills assessments (described below) in two separate sessions. The assessments serve as critical tools to help evaluate the competence of individuals and assist them in gaining insight in all aspects of their daily living.
Once the assessments have been completed, the OTR/L will score the results and produce a report to share with the client and agency staff. At that time, the OTR/L and client will set up goals for the individual to pursue.
Assessments:
Canadian Occupational Performance Measure (COPM): The COPM is an individualized measure designed for use by occupational therapists to detect self-perceived change in occupational performance problems over time. The COPM has three subscales: Self-care (includes personal care; functional mobility, community management); Productivity (includes Paid/Unpaid Work; Household Management; Play/School); and Leisure (Quiet Recreation; Active Recreation; Socialization).
The Independent Living Scales: The Independent Living Scales is an individually administered assessment of adults’ competence in instrumental activities of daily living. The items, which target situations relevant to independent living, require the examinee to do problem solving, to demonstrate knowledge, or to perform a task. Information can be specific enough to identify needed support services, adaptations or instruction for adults who are unable to function independently in certain areas of everyday living.
The Independent Living Scales has five subscales: Memory/Orientation, Managing Money, Managing Home and Transportation, Health and Safety, and Social Adjustment.
Pricing: $175 per person assessed; $225 per session includes two follow-up meetings.
